Description

The Agenzija Appogg Social work service at Mater Dei Hospital is a holistic service to inpatients of all ages suffering from psychosocial difficulties due to their medical condition and situation. This Social Work Service helps patients and their families cope with physical and emotional trauma caused by their admission to hospital. It also assists patients in making decisions and adaptations in preparation for their discharge.

Procedure

Any patient at Mater Dei Hospital may benefit from the Social Work Service by asking the nursing officer in charge of the ward / doctor / consultant / physiotherapist / occupational therapist to contact the social work unit. Furthermore, any professional working at Mater Dei Hospital may refer any of his/her patients to the Social Work Service if s/he deems necessary, after the patient gives his/her consent for such a referral.

Eligibility

Any patient at Mater Dei Hospital may benefit from this service.

Required Documents

A consultation form must be filled by any of the professionals working in the wards at MDH.

Back Office Process

When a patient is referred for social work intervention, the professional referring the patient fills in a referral form. A social worker who works in close liaison with the particular ward from where the referral is received, visits the patients, conducts an assessment, identifies the needs and together with the patient, draws up a plan of action.

Question: Can outpatients of Mater Dei Hospital benefit from this service too?

Answer: Only in-patients can benefit from the service with the exception of referrals from the Ante-Natal Clinic (Outpatients)

Question: What happens when a patient, who would have been receiving a social work service while at hospital, is discharged?

Answer: Following discharge, the social worker keeps following a case for up to a month if need be. If, when at home, the patient would still require assistance, the social worker will refer him/her for further assistance, depending on the particular needs of that person.
